Access Support assists former Commonwealth community mental health clients who need to test their eligibility for the NDIS or do not wish to apply for the NDIS.
Our Access Support Workers with expertise in NDIS applications will assist you to coordinate an NDIS application. An experienced Senior Mental Health Outreach Support Worker then collaborates with you on a Recovery Action Plan for the supports you need during the NDIS access process.
This service is not an emergency response service. However, a safety or crisis awareness plan can be developed with the client.
Eligibility
People where were being actively supported by Partners in Recovery, Support for Day to Day Living or the Personal Helpers and Mentors Service on 30 June 2019 and either need to test for NDIS access or do not wish to apply for the NDIS.
Where is it available?
The Access Support Program is available from our 360 Health Centre Guildford.
Accessing the program
Existing clients are already established. However, if a person is eligible and has moved areas, enquiries can be made to transfer to the 360 service. Find our contact details below.
Please advise if you are an existing Transition Support or Access Support client of a different agency.
