Our Healthy Lifestyle Program supports people with chronic conditions to improve their health and wellbeing and self-manage their condition. Learn lifestyle choices to prevent or reduce the progression of chronic disease and how to utilise community resources to reduce your risk.
This program support people at risk of, or living with
- Type 2 diabetes;
- Cardiovascular disease;
- Chronic renal disease;
- Musculoskeletal conditions; or
- Chronic respiratory disease.
One-on-one appointments and group programs are led by an allied health team, including dietitians, diabetes educators and exercise physiologists. Complementary programs are also accessible to help in sustaining long-term lifestyle changes.
The Healthy Lifestyle Program is provided by 360 Health + Community in partnership with the State Government through the Department of Health WA.
