360 Health + Community is proud to announce a new three-year partnership with the Swan Districts Football Club’s Integrated Football Team and Social Impact Programs.
The partnership sees 360 Health + Community, a leading provider of mental health and psychosocial disability services in Western Australia, continue to expand on their already strong relationship with the Swan Districts Football Club that has been established through their Street Doctor Clinic based at Steel Blue Oval in Bassendean.
The Street Doctor Clinic is staffed by a doctor, nurse and pharmacist, with outreach workers and mental health outreach workers who provide social support to those accessing the clinic. The main aim of the clinic is to improve the health and wellbeing of homeless, transient and disadvantaged people in the community.
The partnership ensures that the Street Doctor Clinic will have a permanent place in Bassendean for the next three years where it will continue to run every Wednesday and Friday.
“This is an important partnership for 360 Health + Community and the Street Doctor Clinic as it allows us to continue to provide an easily accessible, non-judgmental health service to the people who so desperately need it within our community,” said Gautam Gupta, CEO of 360 Health + Community.

360 Health + Community provides a vital wraparound service to people dealing with mental health and psychosocial disabilities as well as those living with physical disabilities. The organisation is an accredited NDIS service provider with expertise in the area of therapy, recovery coaching, support coordination, specialised support coordination as well as allied health services such as diabetes management and education, exercise physiology, speech pathology and occupational therapy.
“While our core strength lies in treating clients with mental health issues, we do also provide care to those with disabilities and why it makes sense for us to partner with the new Swan District’s Integrated Football Team,” concludes Gautam Gupta.
